Form 7-II: Second Opinion of Sickness – Practitioner’s Report
Abstract:
This form may be required by your employing school board to support payment of sick leave benefits and should be returned to your employing school board. The STF Income Continuance Plan provides long-term disability benefits and must be applied for separately from sick leave benefits.
