Saskatchewan School Workplace Violent Incident Reporting Form

Teachers have the right to a safe work environment.

The Saskatchewan School Workplace Violent Incident Reporting Form is now available. It is important that every member is aware of the form and completes it whenever a violent incident occurs.

Why Reporting Matters

Reporting violent incidents is an important step toward:

  • Understanding the scope of violence in schools.
  • Identifying student and classroom needs.
  • Supporting data collection and reporting.
  • Creating a culture of safety in schools.

Reporting incidents is not about blaming students. It is about identifying needs and supporting safe learning and working environments.

What Should Be Reported?

Report all violent incidents that you experience or witness, including but not limited to:

  • Hitting
  • Kicking
  • Biting
  • Scratching
  • Spitting
  • Threats

These incidents must be reported even when the behaviour may seem minor.
